Student Absences Resulting from Medical Situations

Attending Physician must send or fax a typed letter on Physician's or Clinic's letterhead to the Dean of Students office:

The letter must describe:

  1. The medical condition or problem of the student.
  2. Confirmation of date of absence and date of return of the student.
  3. The signature of the attending physician.
