Student Absences Resulting from Medical Situations
Attending Physician must send or fax a typed letter on Physician's or Clinic's letterhead to the Dean of Students office:
Dean of Students

Weatherford, OK 73096
The letter must describe:
- The medical condition or problem of the student.
- Confirmation of date of absence and date of return of the student.
- The signature of the attending physician.
